It has been a privilege to have Dr. Zona as part of our team, and we are grateful for the care and dedication provided to our patients. We understand that continuity of care is important, and we are committed to ensuring a smooth transition for you.

Ongoing Care Options for Dr Zona patients:

Unfortunately, Dr Lepi and Dr Morgan are not able take on all of Dr Zona’s patients due to their patient load and insurance restrictions.

If a family member is current patient of Dr Lepi or Dr Morgan, you may be able to transfer depending on insurance. However, all transfers to Dr Lepi and Dr Morgan will be handled on a case-by-case basis.

Transferring Care Options for Dr Zona patients:

Please see the following list for recommended locations.

We will provide necessary prescriptions for 30 days, after which time refills will not be prescribed by us.

We will be happy to assist with the transfer of your medical records upon receipt of a signed authorization. Medical records requests can be submitted by your new provider’s office or by completing a records release form available at www.buckeyefamilyhealth.com.
